How to align the contents in longtable?
I would like all the three lines of texts displayed by the pdf document generated by the following code to be aligned to the second line of text.

I tried around with option in longtable $indent$, $noindent$ and so on, but can still not obtain the effect I want.
I also want to use $vspace$ to control the spacing between the line with "Personal Data" and "Name". But the result is that although "Name" is spaced from the line with "Personal Data", "abcde" (which should be in the same line as "Name") is not correspondingly aligned with "Name".
How can these two problems be solved?
